Please follow the link below to refer your patient to our program. Our DSMES patient coordinator will reach out to your patients directly to get them enrolled in our next group of sessions!
Program format
what will your patients learn?
During this entirely virtual program, your patient will first meet with our knowledgeable pharmacists one-on-one to complete a comprehensive consultation. Then, participants will attend four group education session covering the topics listed below. At the completion of this program your patient will have received between 9 and 10 hours of direct diabetes education and support.

*Session 1: Cardiovascular health & diabetes
*Session 2: Food & Fitness
*Session 3: Diabetes Complications
*Session 4: Diabetes Medications
Cost to your patient
This service is FREE for patients with Medicare, with a signed order from a provider. The order form can be found within the referral form. Alternatively, you may contact our DSMES program director to have one sent to you via fax or email. See below for Jessica Frasier's contact information.
For participants without medicare the cost of this service is $350.

Jessica completed her Doctor of Pharmacy at the University of Maryland School of Pharmacy in 2020 and her PGY-1 Residency at Tarrytown Pharmacy/University of Texas at Austin College of Pharmacy in 2021. She has a passion for empowering her patients with knowledge so they may take charge of their own health. She is proud to offer this enhanced clinical service to the Austin community and beyond!
